Объекты исследования: первая группа – операционный материал 60 желудков, резецированных по поводу рака желудка кишечного типа, в каждом случае проводили взятие 1 фрагмента ткани опухали и 4 фрагментов слизистой оболочки желудка дистантной зоны (2 – антрального отдела и 2 – тела желудка), вторая группа – 62 случая эндоскопического биопсийного материала слизистой оболочки желудка пациентов с хроническим гастритом с забором 5 фрагментов (биоптатов) ткани по протоколу исследования OLGA-system. Уровень экспрессии miR-21 в тканях определи методом полимеразной цепной реакции с обратной транскрипцией.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что экспрессия miR-21 в образцах дистантной зоны операционного материала при раке желудка значимо выше (Me – 158, p=0.0006) в сравнении с группой эндоскопических биоптатов слизистой оболочки желудка (Me – 42). Наиболее высокий уровень экспрессии отмечался в тканях аденокарциномы (Me – 270), при этом достоверные различия при сравнении его с уровнем экспрессии miR-21 в образцах слизистой оболочкой дистантной зоны отсутствуют (p=0.2). Повышение экспрессии miR-21 позволяет констатировать высокий риск развития аденокарциномы желудка, вероятно, до проявления гистологических признаков неоплазии.</p></sec><sec><title>Заключение</title><p>Заключение. Проведенный для подтверждения предиктивной способности экспрессии miR-21 регрессионный анализ показал увеличение риска ассоциации с аденокарциномой в 13 раз при повышении уровня miR-21 на каждые 50 единиц относительной экспрессии, нормированной на малую РНК U6. Построенная статистическая модель, включающая морфологические критерии оценки слизистой оболочки желудка и экспрессию miR-21, отражающую степень нарушения эпигенетической регуляции молекулярных путей клетки, позволяет повысить прогностическую ценность биопсийного исследования и точность стратифицированной оценки риска развития аденокарциномы желудка при хроническом гастрите.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to evaluate the potential of miR-21 expression in the gastric mucosa as the marker of early stage gastric adenocarcinoma and cancer development.</p><sec><title>Material and methods</title><p>Material and methods. The study included the following materials: the first group consisted of surgical material of 60 stomachs (gastric cancer of intestinal type), in each case, 1 fragment of tumor tissue and 4 specimens of the gastric mucosa of the distant zone (2 – antrum mucosa and 2 – fundal mucosa) were taken; the second group consisted of biopsy material of 62 cases in patients with chronic gastritis (5 specimens according to the OLGA-system protocol). The level of miR-21 expression in tissues was determined by reverse transcription polymerase chain reaction.</p></sec><sec><title>Results</title><p>Results. miR-21 expression in samples with gastric cancer was significantly higher (median – 158, p=0.0006) compared with the group of endoscopic biopsies of the gastric mucosa (median – 42).
